How to Diagnose a Blown Head Gasket (Before You Replace a Radiator, Bearing, or Bulb)

An overheated engine with white steam is not a blown head gasket until a test says it is. I know that sounds cautious, but the cost of guessing goes far beyond the gasket itself. A head gasket usually fails because something else made the engine overheat first, so if you only replace the gasket, the new part is running in the same broken system. Do three checks in order: a cooling system pressure test, a combustion gas test on the coolant, and a compression test. All three can be done with the engine still in the vehicle. If they come back clean, the head gasket is not your problem.

I have a reason for being blunt. I work in Koyo's technical-service group, and my job is looking at returned parts and figuring out what actually failed. Since 2016 I have documented 23 of my own wrong conclusions. Add the shipping, teardown labor, and restocking, and I put the bill at about $46,000. That is not a fun number. In the past 18 months our team's checklist has caught 47 potential misdiagnoses before they became chargebacks, which is why I am writing this down instead of keeping it in my head.

The pattern repeats more often than any particular part failure. The visible symptom feels like a diagnosis. White smoke after an overheat must be the gasket. A growing rumble in a wheel-end must be a bearing. A dead headlight bulb must be a bad bulb. Sometimes yes. But too often the expensive part gets installed before the cheap test gets done.

How to diagnose a blown head gasket, in the order I ask customers to do it

You don't need to disassemble anything for the first two steps. Start cold and work your way toward the engine.

  1. Cooling system pressure test. Let the engine cool completely. Attach a pressure tester to the radiator neck and pump it to the pressure number printed on the cap. Watch it for about ten minutes. If pressure drops and you see coolant on the ground or around a hose, find that leak first. Low coolant from a small leak can cause the same overheating symptoms as a head gasket, and a cheap cracked hose can make you pull a cylinder head for no reason.
  2. Combustion gas test. With the engine warm and running, use a combustion leak tester to draw air from the radiator neck through the test fluid. If the fluid changes from blue to yellow or green, exhaust gas is getting into the cooling system. That points hard at a head gasket or a cracked head. It is not a perfect test, but it beats guessing by a wide margin.
  3. Compression test. Disable the ignition and fuel, pull all spark plugs, hold the throttle wide open, and crank the engine through several compression strokes. Compare the readings to the factory service manual. If two adjacent cylinders are low, a head gasket leak between those cylinders is likely. If only one cylinder is low, add a few drops of oil and retest; if the number jumps up, the problem is more likely rings than a gasket.

The reason this order works is simple: it eliminates the cheapest causes first. If the pressure test shows the cap or a hose is leaking, fix that before you order anything else. If the combustion test is clean and compression is even, the engine is telling you the head gasket is not the failure. Listen to it.

The result that surprises most people is this: a blown head gasket rarely just happens while you are driving. It is usually the damage report from an earlier event. An old radiator tank cracked, the coolant level dropped, the engine got hot once, and the gasket gave up a week later. That is why the original symptom can look exactly like a cooling problem, and why replacing only the radiator does not always end the story.

The same shortcut shows up with parts that have nothing in common. Once you see it, you cannot unsee it.

IS300 Koyo radiator versus a real head gasket failure

The factory radiator on a Lexus IS300 has plastic tanks that age and crack, so many owners decide that an IS300 Koyo radiator is a good planned upgrade. It usually is. But a part order is not a diagnostic plan. I have seen an IS300 Koyo radiator returned as defective because the car still overheated after installation. The radiator was not defective. Pressure held at spec. The combustion tester turned yellow-green in about thirty seconds. The head gasket had already failed because the car had overheated before the new radiator went in. The radiator was doing its job; the diagnostic order was not.

That is not a comment on whether Koyo products are good or bad. It is a comment on when they were installed. If the original radiator cracked and the engine got seriously hot, check for damage before you stack a new part on top of an old problem.

Truck wheel bearing: check for play before you order the hub

Truck wheel bearing complaints sound urgent. There is a low growl that changes with speed, and it is easy to imagine a bearing disintegrating. But a worn wheel bearing usually gives you something easier to check than sound: play. Jack up the wheel, grab the tire at twelve o'clock and six o'clock, and push and pull. If there is no movement and the noise does not change when you turn, look at the tires before you look at the bearing. Cupped or feather-edged front tires growl exactly like a truck wheel bearing, and the fix is tires plus whatever caused the bad wear pattern.

I ate a comeback on this myself in 2021. The noise did not go away because the source was not in the wheel-end at all; it was in a worn suspension part that made the tire wear unevenly. The truck wheel bearing was fine. The expensive replacement was not. Jacking the truck up and wiggling the tire costs nothing. A hub assembly costs plenty.

When a Polaris Ranger headlight bulb is not the problem

Then there is the electrical version of the same mistake. If a Polaris Ranger headlight bulb burns out repeatedly, most people blame the bulb. In my experience, check voltage first. I once watched a side-by-side go through four Polaris Ranger headlight bulbs in two months. The bulbs were fine. The connector had more than 16 volts while the engine was running, which is a charging system fault, not a bulb fault. A $25 multimeter would have found it before the second bulb failed.

What I am not saying

I am not saying parts never fail, and I am not saying you should avoid upgrades. If the engine already ran hot enough to blow coolant out or pin the temperature gauge, do not stop at these simple tests. Get a leak-down test and check the cylinder head for warping. A severe overheat can cause damage that a basic compression test will not fully show.

And if the original radiator is old, replacing it before it fails can be a smart decision even if it passes today's pressure test. Plastic tanks get brittle with age, and there is nothing wrong with planned maintenance. The point is to replace what actually failed and to understand why it failed before you install the next link in the same chain.

Buying a quality part is a decision. Buying it before you know what failed is a gamble. Koyo products are engineering purchases, so treat them that way: verify first, fit second, and you will probably not need to ask the question I answer every week—why the new part did not fix it.
